Exhibit 9-L

This document is a form for those bidding on projects to list all subcontractors, suppliers, and truckers who gave quotes or proposals. It is needed for all requests for proposals (RFPs) and must include both DBE (Disadvantaged Business Enterprises) and non-DBE firms. The form asks for details like firm name, address, DBE status, NAICS code, firm age, and annual gross receipts. It has sections for general project info, prime contractor/consultant details, and subcontractor/subconsultant details. This info does not decide if a bidder is responsive. After the contract is awarded, the completed form must be sent to the Local Public Agency and a copy to the DLAE within 60 days, as per 49 CFR 26.11.
